Park Slot Engaged

I have a site where if calling the main number and choosing option 1 of the auto attendant it rings busy and shows park slot engaged in system status. I have never encountered this before. Any information would be very much appreciated. This is random and not every call.
 
Perhaps more information otherwise we're guessing how the auto-attendant is configured, which voicemail service is being used, the type of IP Office and its operating mode. You didn't even tell us what option 1 does when it works (other than works).

Stuck in a never ending cycle of file copying.
 
IPO is in PBX mode, 11.0
We are also using VM Pro
Option 1 routes to a hunt group with two members.

When you choose option 1 it immediately rings busy although it is not every call. When it does ring busy you can see the status say park slot ENGAGED.

I have never seen park slot ENGAGED and am looking for others who might have and give me some insight.
 
Do any of the extensions (Hunt group, or User extensions) conflict with any shortcodes on your system?
Do any of your users have something in their user shortcode tab, or have call forwarding, or twinning setup?
 
System Monitor app should tell you what's wrong.
Call tab: select Targeting and Voicemail Client.

A successful call with button 1 pressed Should look something like this: (using Action type Transfer , not "Normal Transfer" )
