I can't quite pinpoint how in the world the mitel system would be doing this but it is, any ideas would be appreciated.

System: Mitel 3300 running 6.0
Trunk type: SIP / Carrier is "Clearfly"

successful call:
P-Asserted-Identity: <sip:6194530864@x.x.x.x>
P-Preferred-Identity: "Company Name" <sip:6194530864@192.168.99.5>

Failed call:
P-Asserted-Identity: "F.Lastname" <sip:8584556350@x.x.x.x>
P-Preferred-Identity: "Company Name" <sip:6194530864@192.168.99.5>

Possibly got CPN set to send the name on some extensions? Are they all on the same COS?
