OWA logon page problem

Hello, for some reason our default web access log on screen has dissapeared and we have been left with this:

How can we get it back to how it was?

Your thoughts and suggestions would be

Thanks but tried that, I'm thinking its an SSL related problem.
How can I validate my certreq.txt file so I can allow https connections to the server?

If the cert was expired or untrusted, it wouldn't cause what you're seeing. You'd just a certificate warning.

Looks like a FBA issue - did you used to connect securely or not?

Verify that your IIS security is set correctly.

When you are done verifying the settings and if there is still a problem, run through the forms based auth setup again.
