Outgoing call failure

Mitel 5000 intermittently giving a fast busy on outgoing calls only. Fed by a PRI, no alarms on the T1. The only alarm that is consistent is a off hook busy, to an extension. I can find no other information on this problem.
 
Fast busies are usually a carrier rejecting the call because of a routing failure or invalid number. If the PRI was busy (full), I think by default you camp-on the the Trunk Group until a channel opens up. Maybe someone entered an invalid outbound Caller ID on a station and that is causing the failure? I would start by having them log the number called and extension called from, then test the number's validity.
