I'm not sure if this is your problem, but make sure that you're using the /admin in the URL of the server. The URL that you should be using should be:

If you omit the /admin, you will be directed to the end user login page, which will not accept the server's windows username and password.

give this a try - if not it - give us the exact wording of the error.

Problem Description
Scenario
Getting the following error messages:
Error: Can't create OTMAuthentication Instance

Error: Authentication: 8000401a

Unable to log into the Communication Server 1000 Telephony Manager.
Unable to log into Communication Server 1000 Telephony Manager server using the local admin account.
Cause of problem

The administrative account is not showing logged in as the "Interactive user" on the PC.

Problem Resolution
Perform the following steps:

1) Reboot the PC and log in using the local admin account. Do not attempt to log into the Communication Server 1000 Telephony Manager.

2) Go to Start --> Programs --> Administrative Tools --> Component Services

3) From the Component Services expand the following folders --> Computers --> My Computer --> COM+ Applications

4) Right-click and select the properties of the OTM Application.

5) Select the Identity tab. Change the radio button from Interactive user to This User. Use the browse button to locate the Local Administrators account and select it. Input and confirm the password. Click Apply. Click OK. Log into the Communication Server 1000 Telephony Manager.
