Are you going to connect to the switch via a direct connection (Com port 1 on PC) or via LAN? The OTM login and password; is this after you open up OTM or trying to open OTM? If after ten it is looking for the login and password of your 61C
When you say you are trying to get the server to talk to the Switch via a LAN. The switch has no connection to the CLAN therefore one must use a router to access the ELAN and only allow the communication from the OTM server to the ELAN if you are on the CLAN. Your other option is to put the server on the Elan and route a client PC to the server. As far as determing if you are MAT enabled a this is in reference to if you have switch configured to allow communication. Go to system terminal and go to load 22 and prt adan tty to see if you have any PTY ports built in the switch (Virtual TTY Ports) allowing OTM to communicate with switch. If you don't have any PTY's built you probably are not setup on the switch.

In ld 17 you also have to enable LNAME_OPTION to yes and when building the OTM login name you must also answer yes to the MAT prompt.

There are several things that need to be configured but there is a start for you.
Once you have OTM up and running you have to put usernames and passwords into the OTM configuration. A baby-step would be sure that you can go direct as they are suggesting without OTM and then do the OTM config. Can you ping your PBX from the LAN? We have learned that some things simply don't work if ping is not enabled.
I agree that OTM is a finkiky child-like server (throws tatrums/crashes) but at least I have to still use it for billing, and help me make up for the PBX background that I lack.
