How do I created a Phantom TN that automatically forwards to an outside long distance number?

when you build the 500 set (phantom) put cfxa in the CLS and the ftr would be FTR DCFW 12 91xxxxxxxxxx.

I built it as an ACD. I created it like JohnPoole said in an earlier response. I can go ahead and create a phantom TN. I'm just not sure of the steps to create it. Thanks.
