Open Trunks?

Is it possible that a trunk (ISDN PRI) be left open by the PBX and yet allow a call to
terminate onto that trunk? We had an incident where a trunk "may" have been
in some sort of open state connected to a meet me conference bridge and a
call came through on that line, and all callers on the meet me could hear
the caller (only the internal caller, they could not hear the other party on
that call).

Any ideas what may have allowed a trunk to be kept open like that (and I'm
only speculating that is what happened).
 
Is it possible that the internal caller was talking on a different phone?

Incident:

Caller A was on a conference call, Caller A was using a headset, Caller A thought that they had put the conference on Mute. Caller A made a call on cellphone. Everyone on the conference heard Caller A talking, but was not sure who they were talking to. Salty language was used which resulting in the termination of Caller A.

Maybe you have had a similar expereince.

We think that's what happened but based on the title of the involved party, the truth will probably never be known.
