NT5D12 DB15 (loopback and crossover) 1

I'm having all sorts of trouble integrating my CS1000m with another PBX and I'm looking to verify my pinouts from the Dual PRI card. I'd like to know which pins on the DB15 to cross to make a crossover plug, also which pins should go to what on an RJ-45.

What I've been trying is this:

DB15 RJ
1 1
3 4
9 2
11 5

On the DB15 the pairs are 1,9 and 3,11
On the RJ45 the pairs are 1,2 and 4,5
Make up 2 adapters, so one will work
DB15 to RJ45
1,9 to 1,2
3,11 to 4,5
then make the reverse
DB15 to RJ45
1,9 to 4,5
3,11 to 1,2

Make a loopback plug to test your equipment.
