Notes R5 - address book issues

I have a user that is using Notes R5 and has experienced some strange behaviour with the local and server address books.
Basically what's happening with the local address book is if you go into a contact name to view there details it goes into an 'Account' screen' with details of protcols rather than the usual contact's style screen with the persons information in it. It doesn't seem to do this with every contact entry though. I've tried replacing and refreshing the database design but that had no effect.

With the server address book if the user enters a recipiants name and the system has to clarify who the mail is going to the window that appears has repeated information for each persons name that can be selected. For example it would come up with John Smith,smith,smith,smith,smith and this is all copied when the name is confirmed.

Any ideas??

For the address book try to replace the design template.

Remove the cach.dsk from the domino server

The cache.dsk may be in use while the server is running. You may have to bring down the system to do this. Depends on what version of Domino and what OS you are using.
