Norstar M7xxx Series Feature Card Template

This is a simple question for everyone. Does anyone have a template for the Feature Card (the little sheet of paper under the handset) M7000 series stations. I am not at the office right now so I don't know the actual dimensions of the card and cannot measure it.

I don't see a need for a DESI label either as the card is a simple piece of paper.
