If you do an "inspect" on the SA-O button, what does it show? You may have programmed something over the top of it. If so, go to centralized programming and put it back.
From Centralized Programming, select your System Access-Originate Only button (gernerally button 3) and dial *18.
What may have happened, is if you pressed that button, then selected F10 (list feature) and then hit F10 again instead of 'Page Up' for more. This is a pretty common mistake.
As for the denied list, it takes effect as soon as you hit the enter key. There is no need to reboot the system.

Okay, first go to Extensions-Lines&Trunks and remove pool 60 from your extention, then return to Centralized programming and reprogram your button.
It looks like I somehow turned on the Forced Account Code Entry. I removed my extention from it and now im back up and running. Thank you to everyone that responed to my post.
