New DS30B V2 extensions disconnect/reconnect

I have a new deployment for an existing IP Office 500V2 (running 9.1.12). I ordered a new DS16 V2 and new DS30B V2. The DS16 is working like a champ. THe DS30B is giving me nothing but headaches and ready to get an RMA for it. Basically I have factory defaulted the IP Office using Manager as well as the reset button procedure on the controller with no luck. When you look at the DS30B V2 you will see the extension LED's intermittently blink on/off. Sometimes the phones stay online long enough to make a quick call or check voicemail but when the LED blinks, the phone loses connection and needs to be reset. I see absolutely no errors that I can recognize in System Monitor or Status. It is the strangest thing. Yes I have it set for DS mode (not BS/nortel phones). I have re-upgraded the system again to 9.1.12 as well as factory defaulted SD card and reloaded. At this point I am thinking a defective module?? The new DS16 V2 is working great as well as DS8 cards in main system. Anyone know if anything regarding the DS30B V2 (or aka DS30B2)?
 
Is the firmware for the DS30B present on the SD card?

Can you see it in IP Office manager, and does it show the correct firmware version?
 
That was my first issue (Controller was on version 9.1.8 at which point we found out that the B2 module must have 9.1.12 to work) We then upgraded to version 9.1.12. I even removed the SD card, formatted and recreated the card and performed another upgrade after factory defaulting the system just to make sure. After all of the troubleshooting this is the point where I am thinking the module is defective somehow?
 
You can try to erase the firmware via the serial port.

Same as on a IP Office (at-x)

 
What trunks do you have?

DS modules using the same clocking as PRI/BRI trunks (yes, odd its not affecting the DS16).

Is it connected with the official cable? The traffic is not Ethernet, it is very sensitive to cable length and any wiring faults.

Stuck in a never ending cycle of file copying.
