New cards not working in CS1000 4.5

We added 2x new cards to our system and they dont seem to work properly.

You can IDC the card and it shows XMI001 + XMI002 when you re-seat it but anytime you plug in a 3900 phone the light flashes and no screen or dial tone.

Any ideas? Ive checked the cables and swapped cards.

No clue??
 
Back plane should have a male amphenol cable connector, with a female cable plugged in to it. Count from left to right to make sure you are in the right slot and the associated amphenol is present.
To test out the cable, the easiest way would be to get a male cable, unplug slot 19 cable from the backplane, plug your spare cable in to the cable you just removed, find the blue/white pair on the spare cable and add a tone. Go to your cross connect field and check for the tone. If it is not where you expect it to be, make sure you check all the cables, just in case something was labelled wrong.
 
Changed the cables + the entire second cabinet out last week. Still same issue.

it appears that the phones get power as the display and light goes on then off after plugging the phone in. The screen shows some power (faint blocks) but nothing else. Cant IDU sets, no dialtone, etc....

very strange issue.
