Needed to change DHCP Servers. Phones now not receiving DHCP Leases

Hi,
I don't have a consistent vendor or tech support for my phone system here but recently my domain controller took a dump and it handled DHCP. This wasn't immediately apparent to me but I believe as soon as the DHCP leases expired on the phones they can no longer receive an address. I know how to get around the forms on the PBX and was hoping someone could point me to the right form to change the DHCP server address.
 
Should have mentioned the phones are 5212's and 5224's. They're running off an SX-200_ICP_4.0.0.17 CX

Form 47 has DHCP for the 200
