Need to delete custom form from Exchange

Wizards,

I have a few custom forms that need to be deleted from an Exchange 5.5 Organizational Forms Library. What I've done from Outlook is the following:

Tools / Options / Other / Advanced Options / Custom Forms / Manage Forms

When I select the form to be deleted (or any for that matter), the delete button is grayed out. I'm logged into the domain with an account that has sevice account admin rights at the 5.5 Org level and connecting to a mailbox which lives on the 5.5 server.

It appears that additional permissions are needed? Can anyone assist?
 
Check the client permissions.

/org/folders/system folders/Org. forms library > properties > client permissions button.

Make sure you are specifically listed as an "owner".
 
The permissions will need to replicate. Might need to close and open Outlook as well.
