I have been researching this for about a month now trying to fix the problem. My problem is almost word for word what this thread explains
When I do a test board 01a13 with the loopback I get this

When I do the test on the same board without the loopback, it fails the very first test 138 and instead of error 4, it shows error 8.

I have called Qwest and they say they are good to the smartjack and it's on our end. I have had Avaya out and they say our equipment is good. Well obviously something is not or I am going crazy. Can anybody recommend anything to try here.

Also when I do status signaling-group 4 I get this

STATUS SIGNALING GROUP

Group ID: 4 Active NCA-TSC Count: 0
Group Type: isdn-pri Active CA-TSC Count: 0
Signaling Type: facility associated signaling
Group State: out-of-service

Primary D-Channel

Port: 01A1324 Level 3 State: out-of-service

Secondary D-Channel

Port: Level 3 State: no-link

Thank you for any help anybody can give.

Sounds like your signal group is down. Display your trunk group and see what signal group is assigned to the members. Then do a status signal group X (the number next to the members in the TG) More than likely it will show down.

Yes, I have done that already. If you look above it is signal group 4. The last thing I posted in the original thread shows what I get when I do status signaling-group 4

it does show out of service, but I guess my question is what's next to resolve the issue?

kkattfish,
test number 255 with error 8 means there is a problem with the DS1 interface circuit pack or media module. I am more concerned with the following test
01A13 UDS1-BD 1227 ABORT 1951

This test/erro points to your CSU/DSU. Are you using an intergrated 120A1 or higher CSU? I definately check this first.

Do 'list configuration ds1'. Look for the entry for 01A13. On the right side of the page it will show you if there is a 120A ICSU connected to the board. If there is a 120A CSU, the DS-1 board is not configuted correctly. Do 'change ds1 01A13' and down on the bottom right side change the CSU type from 'other' to 'integrated'. That should solve the Test 138 problem.

I would start swapping "parts" to try and isolate the issue.
cable/ds1 card/csu. If you do that and can not find the issue push back on the carrier.

Thanks for the replies.

Namershard2pick - I believe you are right here after doing what 4meravaya said.

4merAvaya - I did what you stated, and it actually shows "none" for the CSU MOD. I went ahead and did change ds1 01a13 and changed it from other to integrated, but no change. it still shows none on the list config ds1. So it would appear that our csu is the problem?

telecomturner - we have swapped the ds1 board, cable, and ports. but as stated above I think the problem is the CSU

I guess my next question or problem is to figure out where the CSU is as it is an external csu and is not on the back of the server like our other ds1 boards.

After tracing the cable all the way from the board through the ports, it goes directly into a brooktrout TR1034 fax board. So does this mean there is no CSU needed because faxes did work up until about a month ago. Then on Dec. 26th they just stopped working.
I am still showing 2 green lights on the fax board indicating things are correct there. I have also rebuilt that server.

The plot thickens. Does your T1 cable from Qwest terminte to the server, then another T1 cable to your switch? I am not too familiar with the ins and outs of the brooktrout fax board, but I would suspect the problem coulb be with the T1 card in your fax server. Try putting a loop jack on the end of your T1 cable (one that connects to your DS1 board and see if Qwest can loop your equipment. They only tested to their smart jack, but see if they can loop beyond the server. If they cannot, the issue is with your fax server. If they can, the problem is switch or possibly.

roll the transmit and recieve
if it looks good with a loopback
then bad without it
the loopbacks don't care which is the transmit or recieve
the cards do

Have you attempted a restart of the FaxServer. We use a RightFax server and many times when we can't find fault in the PBX, the restart of the far end server does the trick.

My end results.

Ended up not being any of the hardware at all. Turns out that the DS1 card and the signaling-group would only test out okay when I was running the brooktrout diagnostics. Well in the end, I ended up choosing ISDN install for the driver and that seeemed to make everything work. I know for a fact that this is not how it was last time I built this server, but if it works now, then I am happy.

If anybody has similar problems like this in the future and you find this thread, feel free to email me about the problem and I will try to assist you. Thanks to everybody who offered suggestions and tips here, they do not go unappreciated.
