Need script for Mailing list (not long one used on many examples)

I have hotmail as my mail provider. I need a script that looks like the find a forum form field. With a button to the side. This is very much used. The script that I need is for the visitor to add their email address to the form field, then when they hit

You might want to look into using ASP to do this. Your form submission just needs to be added to a simple database. Not too hard.

I am more of a Cold Fusion person, but I know that can be accomplished in under 20 lines of code.
