mount usbdisk on RHEL 4.0 64bit kernel 2.6.9 but unstable

After mounted usb harddisk (ext3 or fat32) and did # ls few times./dev/sdc1 or /dev/sdd1 disappeared after I checked with # fdisk -l. The error message came out "kernel journal commit I/O error"
Any suggestion to solve this problem???

 
Maybe a bad HDD or flash. Does the same device for reliably on other systems?

It could also be a bad usb port or a USB2 device on a USB1 port that needs the extra power draw.

 
Something with the kernel after I used fresh installation linux and can mount succesffuly.Thanks.
