model ? on M3900 phone display

one of my regional offices plugged in a phone to a jack that, at some point in time, had a phone in there. She's getting MODEL? on the display. If my memory is correc, this means the phone is punched down to a pair, but the jack is not cross-connected to a TN. Am I correct? if not, please correct me. Also, when I do an LUVU for this same site for 2000 type TNs I get the below:

why are ports 16-21 showing? this is happing on all the cards. I am showing unused ports 0-8 but also a bunch of 16-31 ports. Why am i seeing these ports? thank you.
 
Here are the details from the patch. Note that is talking about a QPC card but it also works for NT8D02's DGTL LC's. And when you are asking for this patch also ask for the DepLists (A.k.a. Service Packs) to be installed.

LUVU displays data units as spare voice units for QPC578

When using the LUVU command the system shows the data units on the old SL1 cards (8-15)
as spare voice units - but these ports should not be listed.

Setup and actions performed to cause the problem:

QPC578 DD card programmed with 2616 sets. Use the LUVU command to list the spare (unused)
voice units on the card.

Expected Results:

Only spare voice units (units 0 to 7) should be displayed.

Actual Results:

Data units 8 to 15 are included in the display.

Impact of problem:

Very large system - customer cannot tell what is spare and what is not.
