Mitel SIP Trunk connection with ISP 03 IP addresses

I am configuring Mitel ICP 3300 (CXII Controller with MIVB 8.0) to connect to SIP Trunk over Internet from an ISP, ISP provides 3 IP addresses:
- IP servers
- SIP Server Signaling
- SIP Server RTP

In Network Elements Form of SIP Peer and SIP Proxy Added: there is only 1 field (FQDN or IP address:
How to configure Mitel SIP trunk with these three IP addresses?

P/S: In testing phase, I simply connected the Mitel ICP 3300 directly to the internet (test for a short time).

SIP line is just from a local ISP.

We completed the test: SIP Peer with IP server and SIP Proxy with IP SIP Server Signaling

does the isp have a name - do you know what kind of sip exchaneg they are using - there may already be a config guidfe available
