Mitel 5340e IP Display

I have 1 Mitel 5340e IP phone on a MiVoice Business. In the upper left corner of the display, on the same line as the date & time is a picture of a head & shoulders. I have reset the phone to factory defaults, but it's still there. any ideas on hot to get rid of it? Out of 100 phones deployed, it's only on this 1.

That's how mine is, head will disappear after logging out of phone.
