Mitel 5330E No address using Voice Vlan

I am using a Cisco Catalyst 3750X switch on a Mitel system that is not resident to the office. We are using a 3rd party to switch our calls. I can plug the Mitel phones into an unconfigured switch and get an IP address. If I configure the port for a Voice VLAN the phone does not get an address. If I use option 43 or 125 where do I get the sw_TFTP and call_srv addresses from, since we do not have a phone system in the office?
The phone gets an address with VLAN 1 but not with the voice VLAN.
 
I was giving you the interface for the router. This is the VLANs

VLAN 10 10.10.11.1 255.255.255.0
IP-Helper 10.10.10.2 For the DHCP
VLAN 20 10.10.20.1 255.255.255.0
IP-Helper 10.10.10.2 For the DHCP
VLAN 30 10.10.30.1 255.255.255.0
IP-Helper 10.10.10.2 For the DHCP
VLAN 40 10.10.40.1 255.255.255.0
IP-Helper 10.10.10.2 For the DHCP

I setup 4 ports for each VLAN to test that the DHCP was working
Port 1/0/1 is for VLAN 10, Port 1/0/2 is for VLAN 20 ...
Each port is a Switchport Access port.
Any workstation plugged into the 4 ports gets an address and works.
The MITEL gets an add Gateway and I have Option 003 set to the Gateway.
 
Is you gateway in option 3 within the same subnet as the phones. i.e. 10.10.20.0/24
Going by the above it is not. It is set to 10.10.10.10
 
No, I set it to 10.10.10.10.
The router is 10.10.10.10 and the trunk is 10.10.10.9 and the DHCP is 10.10.10.2.
What should it be set to?
 
I changed option 003 address to the address of each VLAN:
VLAN 10 10.10.11.1
VLAN 20 10.10.20.1
VLAN 30 10.10.30.1
VLAN 40 10.10.40.1
And the phone works on all 4 VLANS
Now I have to convert one of then to a Voice Vlan and see if it works.
