Customer: Drs Office
Problem: They require a "panic" button that the patient will be unaware the Dr is pressing. It would ring a predetermined extension and display the extension that is in "panic" mode.

Is this possible without any external equipment?
 
Create a phantom station

program key on doctors phone as secondary line key with no ringing

Program on the other phone as secondary line key with ringing

Name phantom accordingly

Press key.

In pressing the key in a panic situation though would it not be heard by the patient...dial tone and dialing the extension? They want the patient to be completely unaware of the Dr's actions
 
you would get ringback now that you ask.

With that requirement in mind, I'm not sure there is any action that can be taken at the doctors phone that would not make any sound

You can set the ringback tone to silence for anything ringing that device, although when the call is answered it would be on loud speaker.

Depending on the amount of rooms that require the 'panic' button you could wire an SLT port to a toggle switch or button of some sort. This port could be programmed as House Phone (i.e. hotline) to ring reception/phantom. When you short out the SLT pair via the switch it will signal off hook and ring the destination programmed.
When you disconnect the short it will stop ringing so a toggle on/off switch maybe the best solution. Depending on the cabling setup you could wire it close to the outlet so the doctor could bump it with his/her foot.
