MIssing Toolbar in r5 Client

You don't say whether its R5 or R6 but the fix is the same.
R5 On the client got to File-Preferences-SmartIcon Settings
Make sure the Icon bar box (on the right) is ticked.
R6 On the client go to File-Preferences-Toolbar Preferences
In the Basics tab - Toolbar Display, make sure the Show Toolbars box is ticked.
If the above is ok, then it could be the WinNTIconPath entry in the Notes.ini path, by default it usually looks like this WinNTIconPath=C:\Lotus\Notes\Data\W32
R5 On the client got to File-Preferences-SmartIcon Settings
