MICS upgrade problem

I recently added a new module to a MICS 4.0-XC and had a strange problem result. We were putting in a M0x8A analog module into port 9. Powered down the switch, inserted the module and restored power. The analog ports worked fine, did some programming and moved on. Later, the customer reported some phones were not working and upon investigation, I foung 12 dead extensions on Mod 3 (0x16). Went into Maintenance and came up with this:
Mod3: 4 sets
Mod4: 21 sets

In other words, the switch showed 21 sets connected to Mod 4 (also an 0x16) and only 4 sets on Mod 3 even though both were fully wired with 16 phones. When looking at the individual ports (301-316) it showd 305-316 as unequipped. No matter what I did, they would not restore and are useless to this point. Fortunately, they had some spare ports and I was able to rewire, but I need to find out what happened. Any thoughts?

call itas ...probaly should upgrade to a atleast 5.0 to many problems with 4.0
