MICS and startalk flash - VM locks up

I'm having an interesting problem with a Modular ICS with startalk flash voice mail. About 5 times now in 6 months, the customer calls reporting access to voice mail is not working. When they try they get flash busy on the display. The last two times we have found a set that shows Feature 981 on the display and wont change. Unplugging the set and plugging it back in won't clear it. The only clear seems to be cycling power on the KSU and VM. It appears that one set (it is not the set or port, it has been two different users phones now) can somehow lock the VM up and prevent any other access.

Any suggestions where to look for the solution?

Depending on the size of the Norstar (number of users, how many Lines answered by AA) the Flash could be too small. A 2 port flash would only allow 2 people in the voicemail at any given time, and a 4 port, only 4 people at any given time. I don't know that this is your problem, but it is something to consider.
 
4 port voice mail, no one else on the phone, no trunks busy. Not sure what I did when I initialized it, that was a year ago. What would be the concern with initialization with the 7310? It is only my opinion, based on my experience and education...I am always willing to learn, educate me!

Is anyone setup for off premise message notification? I have had on a few occaisions seen where the customer did not have this setup properly and it was unnecessarily trying to use VM Channels for the outbound notification. Then again, you said the trunks are idle so I just thought I would mention it.

My thought would be to reinitialize it and reprogram. If you have a software glitch of any kind it sometimes clears it out.
