So far re-boot gets rid of them, but it will happen once a month. Wiil check if its always the same two

should not need to reboot system just disconnect 1 line and if both drop someone did a conference call

i have a site right now that is having the same trouble ,
i have a smdr on and when i disconnect the lines i can tell what ext setup the conference

the way it can happen is if a set is programed for prime line as pool , they pick up the set (grabs from pool)and press intercom (puts line on hold)hangs up ,picks up set (gets held line) no dial tone so they press intercom and dial 9 or something to get a line and in the process locks the lines together and because there is no connection on 1 line you don't get a disconnect signal

fireeater,
Make sure your lines are not set to Auto Hold. If that is on then the lines act as you say but if you turn that off then the line should only go on hold if it is connected.

this is true but the customer(and i use that term loosely)
always ends up dialing something be for they press the intercom key (go figure)

Have you checked to see if an analog device, ie modem, fax, or alarm, is connected to one of the lines on the system and every do often it is dialing out when you pick a line. How many line are in use when this happens? Count up a see which line it might be.

Had this problem with a local Carrier with analog lines on a T-1. Would have 2,3,4, lines in use with no one on the phones. Problem was the CO which were locking up the lines and they did not rectify. They switched carriers. problem gone. A tech in town had the same problem and I said is it XXX carrier, and it was. That carrier is out of business or has been sold. Don't recall the name. It was over 4 years ago.
