Merlin Extension Problems 1

I have and extension that for one reason or another will always go straight to Voicemail when called. What could cause this? This is a remote switch and I am dialed in with WinSPM 4.0. Any help would b greatly appreciated. Thanks
 
If figured out that the phone had actually gone bad by doing a system response test. Now it works fine except, when the user tries to dial an outside line they get a fast busy. I have check the retrictions and it is set to unrestricted. Am I missing something?
 
Check the FRL of that extensin as well, it has to equal or exceed the FRL of the ARS route needed to go out. TipHelp@charter.net
 
The FRL is set to 3 and so are all the other extensions that are active. All the settings appear to match the working extensions. Any other suggestions? Thanks

 
It was set to Forced account, Everything is working great now. Thanks for all your help.
