Media Error 1

I am getting the following error in the problem report for my backup. What does this mean? What do I have to do?

write error on media id 000003, drive index 0, writing header block, 1117

DOWN'ing drive index 0, it has had at least 3 errors in last 12 hour(s)

cannot write image to media id 000003, drive index 1, The request was aborted.
 
Tape 000003 is bad or Drive 0 is bad normally.

Here is the but part:
Do you run SSO? I see there are two different indexes noted in your error messages. You could have a drive mapping issue as well.
 
I had a similar problem and it ended up being a drive mapping issue. I have an SSO environment. Can you run tpconfig -d? It will show your current configuration. I ran this and it showed two of my drives were down but the java console showed all of my drives were up. My global device database was a mess.
