Macro needed for form filling and sending

Hi All

I have a form in Word that I wish people to complete before pressing a button which will send the completed form back to a specific email address as an attachment.

I have created a macro and toolbar button which saves the form, opens new email and attaches the file but cannot get the email recipient name to prepopulate and then send - can anyone help please?
