M3904 SCR button not working

I have a SCR button 01 that either doesn't respond when pushed or hangs up an incoming call when pushed. I have swapped phones and the trouble stays. Any ideas?

Verified and negated user negligence. (Wish it would have been that easy)

I added ola ira and lpk 1 and still no change.

However, I did find that the culprit here is actually key 00 ACD 8827. Calls in the ACD are being disconnected when she tried to answer with this button.

Yes, she is logged in. ;)

A bad headset can cause a disconnect also.

are you using call forcing, if you are, then pressing that button will disconnect the caller
 
What is call forcing?

In this scenario, Console answers all calls to the business and transfers to appropriate acd group. Members who are logged in here their phone ring and press the associated button for that ACD group. Nothing special that I am aware of.

When this person presses her ACD button, the call is often times disconnected.

I'm stumped...
 
If you press the ACD key while on a call then the call is released. That's how it works. Doesn't matter about call forcing.
If the user presses the key twice, accidentally or on purpose, the call is dropped.
