Lost Icons

I used to get proper individual icons for most of the sites stored in my favourites folder, now they have all defaulted to the standard "blue e". Why is this ? Also, can someone explain SIMPLY how to assign an icon to my own web pages please ?

thanks Jim. I'm halfway sorted. I realise that deleting my temp internet files, removes the favicons for all sites. I've sorted the others now, but lost my icon again. I've uploaded it to my server as 16 x 16 256color but it's not showing
