Losing Hold Music

Has anyone seen issues where the PBX loses the hold music for no reason? This is happening on a 406v2 with 3.0(59). We can reset and reload hold music, but it's gone again after a few hours. Manager (and thus TFTP server) is kept running, so it doesn't seem to be a case of the PBX resetting and not being able to redownload the hold music file.

Any ideas?
 
I am having the same problem using IP400 switch, IP Office 3.o. The hold music plays for 15minutes and then stopps for all users. If you come up with anything, please post.
 
We have experienced this problem.
It was due to a wrong version of phone manager.
Each time a user parked a call from Phone Manager, the holdmusic was down.
Try other version of PM to solve.
