Losing calls at transfer

I am having a problem at a couple sites running 3.0.59 on a 406 v2 were when the receptioninst(4424 with dss) goes to transfer a call when they press transfer the phone does not act right. it does not go to the next call appearance to actually transfer the call. instead the read and green light stay on on the phone and the call is basically lost. sometimes the receptionist is only able to get the call back via the softconsole program. any thoughts?
