Lines Lock up and say "Line in Use"

HELP! I have a 0x32 w/MICS 5.0 I have used this system before with no problems but now installed in a new location it does these things
Lines 1 and 49 and 50 lock up and can not be released. The phones state "Line in Use", not used by extension #. The only way to release the lines is to physically disconnect them.
All these lines show as off-hook at the POTS phone line.
Normally these lines go to voice mail after 4 rings. Now they ring once then are silent.
These lines are CENTREX , could that be the problem?

One thing that it could be. Are the lines set to supervised.If not when the calls are disconnected as in hungup after the call is completed,CO supervision is not detecting that the call is completed.When this happens it will hang the lines on the system.

Already tried changing from Super to Unsuper and it makes no difference. It does it with either setting. I also turned off centrex on lines 49 and 50. I also made sure COD was working.
 
Are the lines in voicemail jail? If you monitor the lines, are they stuck in voicemail? Have you checked to see if you are getting disconnect supervision from Telco?
 
you need to check the connectivity on these lines its sounds like you either have bad cards or the lines are not punched down correctly - Ihad this problem with one of my customers. Hope this helps

with 5.0 you need to disable the card, change lines from unsupervised to supervised, re-enable the card and this will fix problem. when the lines are switched from unsup. to super. and you do line copy this will cause lines to lock up until reboot is done
 
The problem, I believe I am 99% sure is that the Bell company is not providing you disconnect supervision on these lines (Make sure they are loop start) if they are providing disconnect supervision, check with them on the discconect timer and tell them to set it to 460.
