Lines Crossing when two stations Dial at the same time

When 2 stations are dialing on speakerphone simultaneously, as the call is trying to connect the lines switch and the 2 stations get the other phone call. This does not happen if both parties dial while the handset is off-hook. This only seems to happen when both parties are dialing while using the handfree. I can hear the lines clicking over when it is connecting the call. any ideas as to what this can be would be really great.

Never have seen that problem, but a re-boot will usually cure strange happenings.

does the problem happen every time when dialing in HF mode?

what happens when you dial in HF and then right away pickup the handset?
 
Does it make a difference if you mute the mics prior to dialing???
