Line Ringing Busy, LHK is 5

I can't figure out why this TN rings busy after 2 of the 5 lines are engaged. Can anyone see a reason why it would do that? It doesn't even go to voicemail. Any help would be great. Thanks! TJ

then something is wrong in the IDC. Put 8787 pointing to 1454. The carrier usually outpulses that last four digits, and doesn't often change that.

I figured out what is going on finally! 8787 was being forwarded manually to 8177 from an Avaya switch across the street. Turns out that the 8787 DID does not even terminate into the Nortel switch. I'm going to try to call the 8177 from outside and see if I get the same problem. I'm guessing I won't. Thanks for all your help!
