LG Aria conversation cut off by second caller

Today we have noticed a weird problem.
Receptionist was making an external phone conversation. During conversation an internal client cut off the line and start talking to the receptionist. Receptinoist also surprised and asked to the internal client how she did it. She said that receptionist phoen was engaged and she pressed the star (*) button twice. Then internal client hanged up by request of receptionist and continued the conversation with external call.

My question is, how it happened and how can I prevent same thing happen again?

Check the pick up group and auto pick up settings or take out the receptionist extension to a pick up group.
 
Receptionist extension is not in pick up group.
 
Not having manuals around here, but check a function called "barge in
 
Barge in not available in any menu option :(
 
under pgm 112 make sure camp on is off for that ext or all of them also make sure pgm 113 over ride privilege is off
