I have a PBX on a subnet where there is no internet service, somehow accessed within the PBX and made international calls.The pbx does not have disa,modem or sip license.

The client also has a voice mail kx-tva200 there certain vulnerability in the voicemail?

 
Yes it is a long time known that you have to put good passwords on managers mailbox or disable access to it through the pc tool
They also change the pass word to slow the changing back the fwd they set to prem numbers or the mailbox they set external delivery to prem lines
Lock down the vm ports to stop it making calls and block external fwd to extensions that don't need it
