KX-TA824 voice mail issue

I have had this system operational for 4+ years now with the built in voice mail feature working flawlessly. Not sure what has happened (other than someone pushing buttons on the Manager phone).. when an outside call is answered by the automated attendent one can select an extension. the extension will rinig 2x upon which all phones in the office will ring 2x more and then the call will be dropped.. The caller never gets a mailbox. I am certain it is something silly however I ahe been through the books and software and have found nothing.

ANY suggestions?

make sure all the messages are cleared in the vm, if it is full, it will not answer any calls. one extension could cause this
 
Thanks obtsystems. I have check and there are only a few voice mails stored throughout the office. total of 4 pohnes. also looked at all the forwarding and seems to be correct. thought may be something with forwarding as the extension rings 2x then all phones ring....
 
is the system in night mode? or if they could get in to main programming could they have changed intercept
