IPO TCM module randomly freezes, the system status shows it recognizing the phone device, yet dead

The IPO is running release 10.o software. Randomly ( several months pass) then the TCM - Norstar module in slot 2 freezes. The system status posts its detecting the associated phone device. However the phones on this module are dead, have no display and do not respond to being called or placing a call. The system has 3 TCM modules in slots 1, 2, and 3.
I have replaced the TCM module and system Cabinet. Problem still occurs over the passing of time. Current resolution of problem is to perform cold restart of IPO controller.
At this time Avaya support has no suggestions. the monitor trace does not show any clues.
Wondering if anybody has experienced this problem? Thoughts and suggestion are appreciated.

It is possible that one of the TCM phones themselves is bringing down the module.

Are the leds on the module out or is there any life in the module

Are any of the components showing burn marks.

Does the module have a child card - analogue perhaps.

You could move all of the users and cables into a module on slot 4 and see if it reoccurs

 
Are you sure you have _supported_ TCM phones? Not all Norstar digital phones are supported. There is some variants you need to watch out for. The phones are the only remaining part that is consistent from start to finish, everything else has been replaced. I would be looking at the phones and features programmed against (on) them.
