IPO: EMBEDDED VOICEMAIL TYPE AA GREETING IS GARBLED & DISTORTED

I have a customer with IP Office 500v2 10.1.0.1 build 3 systems installed and they use the embedded VM for their Auto Attendant. Several times the AA greetings will become distorted (almost sound garbled and robotic when you call into them). During this time there will be an alarm in System Status that says 'All VM Channels in use' even though there are currently no active calls. The call will eventually overflow to the Fallback destination and the call is fine at that point, both parties can hear each other. A reboot resolves the issue but may or may not come back (days or weeks later).
I re-created the SD Card which appeared to 'fix' the issue...or so we thought. Currently,the issue keeps returning. I'm also seeing where the VM channels under System resources in System Status is showing 100% usage when in fact, there are no active calls present.

I can't find much information on this when searching either the forums or the internet but hoping someone has had this issue before and may know exactly what is happening.

Any help would be appreciated!

I would 1st save a back up of the config, then I would recreate the sd card from manager and go from there.

So you are suggesting re-creating the SD card again?
 
Oh sorry I did miss that in your post, then if you have IPOSS Avaya coverage then open up a ticket with them.
 
I have seen this in the past.
A call comes in to the IPO (AA) which transfers the call out and one channel stuck.
Next call same thing happens and all channels are full and the next call sounded garbled.
I think it was 6.0 or 6.1 but i cannot find it anymore.
Is this the same problem?

Yes you are correct and if so, still isn't fixed in r10...would be great to find it
