IPO 500v2 Expansion ports dead 1

Never had this happen before. Two DS 30 units on ports 3 and 4 down this morning. Swapped them with new ones and still dead. Moved both modules to expansion ports 5 and 6 and they came up.

Could the ports 3 and 4 have been damaged or blow out some how?

I've had this happen before. Are your expansions & V2 grounded and how old is the V2 system?
