IPO 406 4412D phones disconnecting

i have been struggling with an IPO 406v1 with approximately 30 users, connected to DS30 and DS16 expansion stations. About 15 times a day, 90% of the phones will lose their display, the voicemail light blinks, and then it goes back to displaying name/ext. We are on release 3.2.69. I stabilized the system about a month ago by restoring a configuration which was 2 months old. However, since that time, there have been some new hires and i needed to add them to the phone system. when i added the new users, the issues came back. The users were added from existing extension from people who had left the company (just changed their names on the system from Extxxx to User User); when this happens, if the user is on the phone, obviously, the call gets dropped; i have uninstalled and reinstalled the manager application and the issue persists. If anyone has any suggestions, please help. Thanks!

Use external power supplies for the phones.



1. A maximum of twenty-seven 4412D telephones are supported on the DS30 (version 2) expansion module at PCS level 5. Earlier DS30 expansion modules will only support sixteen of these telephones.
2. A maximum of twenty-four 4424D telephones are supported on the DS30 (version 2) expansion module at PCS level 5. Earlier DS30 expansion modules will only support sixteen of these telephones.
3. DSS4450, XM24 and EU24 requires external power supply to the Telephone set it's attached to.
4. IP Office supports a maximum two 4450 units on each Digital Station expansion module, including the 406 V2 control unit.
5. IP Office supports a maximum of eight EU24/EU24 BL's on each IP Office system.
6. 9040 Digital Wireless System (dual radio module TDL/DCP only).
7. IP Office supports a maximum two XM24 units on each Digital Station expansion module, including the 406 V2 control unit.
8. The IP Office control units and expansion modules are designed to be stacked free-standing. However they can also be rack mounted using optional rack mounting kits. The Small Office Edition cannot be rack mounted but can be wall mounted if required.
9. Phone (V2) Modules are supported by IP Office Software Release 3.0(44) onwards and also by the August 2005 Administration Maintenance Release Software R2.1(44).
10. DS (V2) Modules are supported by IP Office Software Release 2.1(31), 3.0 and 3.0(DT)

exactly how do i go about securing an external power source for these phones, and where do you plug it in? on the bottom of the phone it states "Use only with communication circuit power source!" there is a slot on the side to plug in the expansion module for the receptionist station, but i dont see anywhere else to plug in power.

The old Magix 4412D/4424D take more juice than the 64xx Phones. Too many of them will cause the digital module to reset buy itself. If that DS30 is v2, do not occupy more than 24 of the ports. If it's an older DS30, stick with 17 or less 44xx Phones. Or You could replace some of them with 6416D/6424D as they are very cheap now, and are very reliable.

Thank you so much for your replies. I am now in the process of installing additional 16 port DS to accommodate growth and to remove some of the phones from the DS30. The DS30 is a V1, and according to the documentation that they provided me, the V2's had issues with line power, not the V1. the tech tip is 143 that they provided. Their explanation is that theses disconnects (and i mean literally, there is no display on the phones) is coming from timing issues at the CO. we have had the lines tested multiple times and the carrier has found no issues with the line. i had to provide the link to the tech tip before avaya even acknowledged that this could be an issue. do any of you have links to technical documentation that would point us to the V1 having issues?
