IP Office rebooting 1

IP500 V2 11.0.4.2.0

IP Office is randomly rebooting about once a day. Status shows DSI Exception with different data each time:
(Abnormal Termination, Cause: <DSI Exception> addr=00000018 d=5 pc=f02721b8 stack=f192cc58 f078c8a4 f0270f34 f078d418 f078fa28)

I turned on a bunch of things in Monitor, and it shows below:

Any suggestions? Any thoughts on more / different traces to turn on in Monitor to try and get more info?

First step is upgrade. There have been five R11.0.4 service packs since the release you are running was made available 3 years ago.

Stuck in a never ending cycle of file copying.
