IP OFFICE _Digital Expansion Module 1/30 ports working

Digital Expansion module 30-port (DS-30B) 700501586 -- Expansion module was working on IPO 9.1 at client’s location since installation for the last ten (10) months. Upon arrival and troubleshooting the issue, it was found that the Digital Exp. Module had only 1 of 30 ports working. What could be the possible cause or causes for this to occur and what can be done to prevent further reoccurrences.
Client indicated that this happened over a weekend and no one was in office to verify if there were any power fluctuations and/or bad weather.
N.B. – The Expansion module was grounded and on a APC 1000 VA SmartUPS.
 
Unplug all of the stations and put them back one by one. Could be that someone plugged an ethernet adapter into a digital port somewhere. Although that would usually turn the LED on the module red, its worth a shot if you haven't done so already. Have you tried the obvious of replacing the expansion module with one from your stock?
 
I did remove all the stations and took one phone and plugged it into each port...all 2-30 didn't come up only port 1!!! - which is the STRANGEST thing...
Presently, we are out-of-stock of the expansion modules, awaiting a new shipment.
 
Have you tried replacing the power adaptor..it may not gave enough output to properly power up the unit components.
 
Snowman50: Not yet...however, my thought is I disconnected all the phones and individually checked one-by-one all the ports and only port 1 came up all the rest were down!
