ip office 500 PRI running v4.07

I am running ip office 500 with a full 24 port pri. with normal used the calls flow well around 14 channels at high usage. We have added conferencing and when the system gets around 15 to 20 calling in and connected , I hear a long tone and Pri goes down causing all calls to be dropped. The PRI does not show out of service, but Once this happens no calls can com in or go out. I call the phone company and they auto test the line and it comes back up. The phone company runs a stress test and they say it is clean. I turned on the look back feature on the ip 500 on and they run tests to the csu it also runs clean. I can at any time start filling up the channels and get the problem. Any thought on this issue could it be the iip 500 pri card?
 
I heard the beep from the phone receiver. and only on the local phone connected to the ip500 in house the outside users et dropped off.
 
Avaya replaced the PRI card and it is fixed. thanks for the help.
