I have had many quirky problems with the AUDIX MWI, if nothing else works, my usual solution is to delete the user station, delete the mailbox subscription, replace the phone cable between jack and phone, and then recreate the mailbox, extension, and let the user know they will have to re-configure their voicemail box.
This seems to clear the problem 99% of the time, surprisingly enough; albeit its a user imposition.
