Intertel Axxess Off Hook notification

Picked up maintenance on an Intertel Axxess recently and customer says that the guard shack superset phone receives notifications if a phone is off hook. An off hook notification keeps popping up about a phone that isn't off hook.

I am used to working on the Mitel iteration of this, the 5000.

My questions are: Where in the system can you program off hook notification?

What would cause a false notification?

I have installed many 5000s but I have never set up any off hook notifications...

The guardshack is flagged as an admin, thats why its gets the notification.
You can uncheck this under the user settings.
The offhook alert could be legit,,,
User didnt hang up the phone before re-order/timeout kicked in,,,generates a log entry, notifies the admin.
You could probably match this event up in the maintenance logs.

Other offhook alarms where based on bad batch of phones (4xxx) series with bad hookswitches....

I've seen headset lifters that need a bit of adjustment because they don't always hang up all the way cause this intermittently. The offending extension shows up in msg print as Makkons mentioned, and should show up on the guard shack's lcd as well.
